Mechanism of action: It is a potent NSAID with strong anti-inflammatory, analgesic and antipyretic effects. It selectively inhibits cyclooxygenase-2 (COX-2) to block the synthesis and release of prostaglandins in inflammatory tissues, relieves pain and inflammatory swelling, and acts on the hypothalamic thermoregulatory center to reduce fever rapidly. Its anti-inflammatory effect is stronger than ibuprofen and naproxen, analgesic effect is fast and lasting, and the onset time is within 15 minutes after injection.
Indications: Relief of moderate to severe pain caused by rheumatoid arthritis, osteoarthritis, ankylosing spondylitis and gouty arthritis; treatment of acute musculoskeletal pain such as severe myalgia, neuralgia, lumbago, backache, sprain, contusion and fracture pain; emergency relief of severe headache, toothache, dysmenorrhea and postoperative severe pain; antipyretic for persistent high fever caused by severe infection and inflammatory diseases; anti-inflammatory and pain relief for acute soft tissue inflammation, tenosynovitis, bursitis and pericarditis.
Usage and dosage: Intramuscular injection / slow intravenous drip (diluted with normal saline). Adult: 50mg (2ml) once a time, 1~2 times a day; severe pain/inflammation: 75mg (3ml) once a time, once a day, maximum daily dose ≤150mg. The course of treatment is 3~7 days for analgesia and fever reduction, 7~14 days for anti-inflammation; adjust dosage according to symptoms strictly under medical guidance.
Contraindications: Contraindicated in patients allergic to this product, diclofenac sodium, NSAIDs and any excipients; contraindicated in patients with active peptic ulcer, gastrointestinal bleeding/perforation history; contraindicated in severe hepatic/renal/cardiac insufficiency, severe heart failure and hypertension crisis patients; contraindicated in pregnant women (third trimester), lactating women and minors under 18 years old; contraindicated in patients with severe blood system diseases, coagulation dysfunction and aspirin-induced asthma.
Cautious population: Use with caution in mild to moderate hepatic/renal insufficiency patients (monitor liver/kidney function regularly, reduce dosage); use with caution in gastrointestinal disease history patients (avoid long-term use, prevent bleeding); use with caution in hypertension, edema and heart disease patients (may cause fluid retention and blood pressure rise); elderly patients with reduced organ function take the minimum effective dose; allergic constitution patients use under close observation; avoid combined use with other NSAIDs, antipyretic analgesics and alcohol to prevent severe liver injury and gastrointestinal bleeding.
Adverse reactions: Common reactions: Mild local pain, swelling and induration at intramuscular injection site; occasional dizziness, headache, fatigue and mild nausea, which relieve spontaneously after drug withdrawal. Rare reactions: Transient elevation of transaminase and renal function indexes; mild gastrointestinal discomfort (abdominal pain, diarrhea); skin rash, pruritus and other allergic reactions; peripheral edema and palpitations in individual patients. Severe adverse reactions: Extremely rare severe anaphylactic shock, gastrointestinal bleeding/perforation, severe liver/kidney damage and thrombocytopenia; adverse reactions are mild and controllable with standard dosage, and the incidence is significantly lower than oral preparations.
Notes: Strictly avoid rapid intravenous injection to prevent hypotension and arrhythmia; intramuscular injection should be deep into the gluteal muscle to reduce local irritation; do not mix with alkaline drugs in the same infusion bottle to avoid drug precipitation and inactivation; long-term medication should monitor liver function, renal function and blood routine regularly; stop medication immediately and give symptomatic treatment if gastrointestinal bleeding, rash, jaundice or oliguria occurs; store in a cool and dark place, avoid light and high temperature, seal for preservation
