Mechanism of action: Inhibits the reabsorption of sodium, chloride and water in the ascending branch of the loop of Henle in renal tubules, increases the excretion of water, sodium, chloride, potassium, calcium and magnesium, produces strong and rapid diuretic effect, takes effect in 5 minutes after intravenous injection, reaches the peak in 30 minutes, and the duration is 2-4 hours.
Indications: Used for edema diseases including cardiac edema, renal edema, hepatic edema, pulmonary edema and cerebral edema; acute pulmonary edema and acute cerebral edema as first choice; acute renal failure with oliguria and anuria; accelerated excretion of toxic substances in acute drug poisoning; hypertension complicated with heart failure or renal insufficiency.
Usage and dosage: Intravenous injection or intravenous drip. Adult: 20~40mg once, inject slowly intravenously, repeat the dose after 1~2 hours if necessary, the maximum dose is 100mg per time; intravenous drip: 20~40mg added into normal saline or glucose injection, the infusion rate should be slow. Pediatric: 1~2mg per kg body weight each time, intravenous injection, adjust the dose according to the condition.
Contraindications: Contraindicated in patients allergic to this product and other sulfonamide diuretics; contraindicated in patients with anuria or severe renal failure without renal perfusion; contraindicated in patients with hepatic coma; contraindicated in patients with severe electrolyte disturbance.
Cautious population: Use with caution in patients with heart failure, severe liver and kidney dysfunction, diabetes, hyperuricemia/gout, hypoproteinemia; use with caution in pregnant and lactating women; use with caution in elderly patients with reduced renal function; use with caution in children and adjust the dose according to body weight.
Adverse reactions: Common electrolyte disturbance such as hypokalemia, hyponatremia, hypochloremia, hypocalcemia; water and electrolyte imbalance such as dehydration and hypotension; gastrointestinal discomfort such as nausea, vomiting, abdominal pain and diarrhea; rare tinnitus, hearing loss (reversible), rash, pruritus, elevated blood sugar and blood uric acid; long-term use may cause renal function damage and blood lipid disorder.
Notes: Strictly monitor electrolyte, renal function and blood pressure during medication; supplement potassium in time to prevent hypokalemia; avoid combined use with ototoxic drugs such as aminoglycosides to prevent hearing loss; the diuretic effect is strong, avoid excessive diuresis leading to shock; it is forbidden to mix with acidic drugs for intravenous drip, and it should be used alone.
