Pharmacological category: Aminoglycoside Antibiotic, broad-spectrum antibacterial drug
1.Mechanism of action:
It is a potent broad-spectrum aminoglycoside antibiotic with strong bactericidal effect. It inhibits the synthesis of bacterial protein by acting on bacterial ribosomes, destroys bacterial metabolic process and reproductions, and exerts antibacterial activity against most Gram-negative bacilli and partial Gram-positive bacteria. It has strong antibacterial effect on drug-resistant strains and is not easy to produce cross-resistance.
2.Indications:
Treatment of severe bacterial infections caused by sensitive bacteria, including respiratory tract infection, urinary tract infection, biliary tract infection, abdominal cavity infection, skin and soft tissue infection, sepsis and septicemia; adjuvant treatment of osteomyelitis, endocarditis and neonatal bacterial infection; anti-infective treatment for postoperative wound infection and burn infection.
3.Usage and dosage:
Intramuscular injection / slow intravenous drip. Adult: 80mg (1 ampoule) once a time, 2-3 times a day, the maximum daily dose ≤ 320mg. Pediatric: 2-4mg per kg body weight per day, divided into 2-3 times for administration. The course of treatment is 7-14 days, and the dosage should be adjusted according to renal function strictly.
4.Contraindications:
Contraindicated in patients allergic to this product, aminoglycoside antibiotics and any excipients of the preparation; contraindicated in patients with severe renal insufficiency and auditory nerve damage.
5.Cautious population:
Use with extreme caution in patients with renal insufficiency (monitor renal function and adjust dosage); use with caution in patients with auditory nerve dysfunction (avoid ototoxicity); use with caution in pregnant and lactating women (may affect fetus/infant hearing); elderly patients with reduced organ function use small dose and shorten the course of treatment; infants and neonates use strictly according to body weight to avoid drug accumulation; avoid combined use with other ototoxic and nephrotoxic drugs.
6.Adverse reactions:
Common reactions: mild local pain, swelling and induration at intramuscular injection site; occasional dizziness, headache and fatigue. Ototoxicity & Nephrotoxicity: rare mild tinnitus, hearing loss and renal function damage with conventional dosage (reversible after drug withdrawal), severe toxicity occurs only in long-term large-dose use. Allergic reactions: skin rash, pruritus are occasional, severe anaphylactic shock is extremely rare. The incidence of adverse reactions is low with standard dosage, high safety for clinical anti-infection treatment.
7.Notes:
Do not mix with other antibiotics and acidic drugs in the same infusion bottle to avoid drug inactivation; slow intravenous drip is recommended to prevent hypotension and adverse reactions; regular monitoring of renal function and hearing during long-term medication; stop medication immediately if tinnitus, oliguria or allergic symptoms occur; store in a cool and dark place, avoid light and high temperature, seal for preservation.
