Pharmacological category: Anti-anemia drug,iron supplement preparation (colloidal iron-dextran complex)
1.Mechanism of action:
Supplies elemental iron to the human body, iron is the core component for hemoglobin synthesis, participates in the formation of red blood cells and oxygen transport; the dextran macromolecule chelates iron ions to reduce free iron irritation, the colloidal complex is slowly released and absorbed after intramuscular injection, prolongs the effective time, increases serum iron and ferritin levels, and corrects iron deficiency anemia fundamentally.
2.Indications:
Severe iron deficiency anemia that is ineffective for oral iron supplementation, including anemia caused by chronic blood loss, severe malnutrition, late pregnancy, postpartum lactation and chronic consumptive diseases; iron deficiency anemia with severe gastrointestinal intolerance to oral iron agents; clinical emergency requiring rapid iron supplementation to raise hemoglobin levels.
3.Usage and dosage:
Deep intramuscular injection (gluteus maximus) is preferred, slow intravenous drip is available under strict medical supervision (for severe anemia). Adult: 125~250mg (2.5~5ml) once, once a day or every other day; the total iron demand is calculated according to hemoglobin value to avoid overdose, the cumulative dose should not exceed the therapeutic total amount. Pediatric: 5mg per kg body weight (elemental iron) per time, once a day, adjust the dosage according to age and anemia severity, strictly avoid excessive administration.
4.Contraindications:
Contraindicated in patients allergic to this product, iron preparations and dextran components; contraindicated in patients with iron overload, hereditary hemochromatosis and hemosiderosis; contraindicated in patients with various hemolytic anemias (autoimmune hemolytic anemia, sickle cell anemia); contraindicated in patients with severe hepatic and renal insufficiency, acute active infection and severe inflammatory diseases; contraindicated in patients with untreated peptic ulcer and active gastrointestinal bleeding.
5.Cautious population:
Use with caution in patients with mild to moderate hepatic and renal dysfunction (regularly monitor liver and kidney function); use with caution in patients with a history of allergic diseases (asthma, urticaria, allergic rhinitis) due to higher hypersensitivity risk; pregnant and lactating women use after weighing the benefits and risks, avoid large-dose long-term use; elderly patients with reduced organ function use small dose and slow administration; infants and young children use strictly according to body weight to prevent iron poisoning; patients with cardiovascular diseases use with caution to avoid increased blood viscosity.
6.Adverse reactions:
Local reactions: common pain, swelling, induration and skin discoloration at the deep intramuscular injection site, sterile abscess may occur in a small number of cases (relieved by hot compress), the incidence is slightly higher than low-dose specifications. Systemic reactions: mild nausea, vomiting, abdominal distension, constipation and metallic taste in the mouth; occasional dizziness, headache, fatigue, low-grade fever and myalgia; rare allergic reactions (skin rash, pruritus, urticaria), severe anaphylactic shock (rare but life-threatening, need emergency treatment); long-term excessive use can cause iron overload, hemosiderosis, elevated serum transaminase and liver function damage; black stool is a normal phenomenon of iron metabolism, not gastrointestinal bleeding.
7.Notes:
Must be administered by deep intramuscular injection to minimize local irritation, strictly avoid intravascular injection; skin allergy test is mandatory before the first medication, stop the drug immediately and give symptomatic treatment if allergic symptoms occur; calculate the total therapeutic iron dose accurately to prevent iron poisoning; do not mix with other drugs in the same syringe or infusion bottle; regularly monitor serum iron, serum ferritin and hemoglobin during medication to evaluate the curative effect; stop oral iron supplements during injection treatment to avoid excessive iron intake; store in a cool and dark place, avoid light and high temperature, shake well before use.
